I am trying to have my biological father name put on my birth certificate and have this other guy name took off because I don't know him never seen him and if something happen to me or my biological father it won't be any questions about anything. The guy name was automatically put on my birth certificate just because the laws was different when I was born and at the time my mother was married but separated from this guy. Please change my birth certificate and my last name on my birth certificate.
