Further, in receiving and/or observing treatment from Dr. Cole, it has become apparent that his current office facility located at 5922 Cattlemen Ln., Suite 203, Sarasota, FL is insufficient to provide for Dr. Cole's current patient load, restricting Dr. Cole's ability to provide medical treatment to all of his patients.
As patients and colleagues of Dr. Cole we hereby request any and all interested parties restricting Dr. Cole's ability to relocate, to allow for the relocation and to permit Dr. Cole's current medical practice to move to 4701 Bee Ridge Road, Sarasota, FL, effective immediately.
